2022-2023 Activities Registration Online!
Please read and follow the steps below carefully.
IMPORTANT: You are only registered once ALL steps are completed.
Step 1: Physical Examinations
Students registering for Athletics must have a current sports qualifying physical exam on file in the Activities Office before the student is authorized to participate in practice. To ensure clearance for participation, physical should be completed and turned in to the Activites Office no later than 3 school days prior to the first day of practice. Click HERE for a current MSHSL physical form. Physicals are good for 3 years from date of your last physical. Once you have your sports physical completed, you can turn it in to the Activities Office or you can scan it and submit completed Physical Forms online.
Step 2: Fill out and submit the online registration form. It would be helpful to have medical information such as doctor, dentist, and emergency contact numbers near you when registering.
Step 3: Upon completion of your registration form visit RevTrak to pay with a credit or debit card. To pay with cash or check, please bring payment to the Activities Office. Checks are payable to ISD 743. If you qualify for the Free or Reduced Lunch Program, please bring your payment into the Activities Office.
STUDENT ACCIDENT INSURANCE: If you would like additional insurance coverage or if your child does not have medical insurance please visit Student Assurance Services, Inc. and select the insurance plan to help offset the cost of medical care.
